Alyson Pelan is a dedicated and experienced Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC) with over 8 years of clinical experience in primary care, long-term care facilities, and specialized patient management. Alyson has developed a comprehensive skill set in patient assessment, chronic disease management, and interdisciplinary care, with a strong focus on improving patient outcomes through compassionate and evidence-based care.
Currently working for CHP Alyson provides Primary Care to CHP clients in the Western & Northern NE communities including Norfolk, Columbus, Neligh and many more. Alyson has served as the Interim Director of Nursing at Westfield Quality Care in Aurora, NE, Alyson leads nursing operations, quality assurance initiatives, and regulatory compliance to ensure the highest standards of patient care in a long-term care setting. She has successfully directed restorative nursing programs and overseen mental health and wound care management, always striving to improve patient outcomes through collaboration and performance improvement.
Previously, Alyson has held leadership roles such as Director of Nursing at Mid-Nebraska Lutheran Home and has demonstrated expertise in risk management, infection control, and policy development. Her hands-on approach to patient care is complemented by her experience as a Nurse Practitioner, where she provided primary care in skilled nursing facilities and home health settings. She also has specialized experience in men’s health at Limitless Male Medical Clinic, focusing on hormone imbalances, preventive care, and wellness promotion.
Alyson’s leadership extends beyond clinical care to business management, having founded and operated Rise Above LLC Home Health, where she was responsible for all aspects of home health care operations, including billing, staffing, and community partnerships. Her work in home health further reflects her commitment to delivering client-centered care and supporting holistic patient health in the home and community.
Alyson holds a Master of Science in Nursing from Spring Arbor University and is board-certified as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC). She also maintains certifications in Advanced Cardiovascular Life Support (ACLS) and is licensed to practice as a Nurse Practitioner and Registered Nurse in Nebraska.

Andrew "Andy" Lundstrom, APRN-BC, AGACNP
"Andy" Lundstrom, APRN-BC, AGACNP, is a nurse practitioner with significant experience in acute care internal medicine. He received his bachelor’s degree at the Bryan College of Health Sciences in Lincoln, NE, majoring in nursing sciences with a minor in healthcare management. He completed his master's degree at the University of Nebraska Medical Center’s College of Nursing in Omaha, NE.
Andrew is board-certified as an adult gerontological acute care nurse practitioner. Prior to joining Community Healthcare Partners, he worked with Inpatient Physician Associates on the hospitalist team at Bryan Medical Center in Lincoln, NE. Before completing his master's, he worked for eight years in the cardiovascular ICU at Bryan Medical Center and as a ventricular assist device coordinator for Bryan Heart. He also served as a graduate assistant and was a clinical instructor for undergraduate nursing students.
Ashley Lundstrom (Watsek), APRN, ACNP-BC
Ashley Lundstrom (Watsek), APRN, ACNP-BC, is an experienced Nurse Practitioner with a strong background in pulmonary care, rehabilitation medicine, inpatient clinical management, and telehealth-based chronic disease support. She currently serves as a Nurse Practitioner with CHP. Ashley worked at Teledigm Health in Lincoln, Nebraska, where she provided high-quality, patient-centered care across multiple rural and critical access hospital partners throughout Nebraska and Iowa.
Ashley is board certified by the American Nurses Credentialing Center (ANCC) as an Acute Care Nurse Practitioner. She also maintains certifications in Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS), reflecting her dedication to maintaining the highest clinical standards in acute and complex care environments.
Ashley began her advanced practice career in 2013 at Nebraska Medicine and has since held key roles with Inpatient Physician Associates, Nebraska Pulmonary Specialties, and Madonna Rehabilitation Hospital. Her diverse experience ranges from pulmonary and critical care management to inpatient medicine and post-acute rehabilitation. Since joining Teledigm Health in 2020, she has expanded her expertise in telemedicine, remote patient management, and integrated care coordination.
Throughout her career, Ashley has collaborated with numerous healthcare systems across the region, including Box Butte General Hospital, Bryan Medical Center, Grand Island Regional Medical Center, Crete Area Medical Center, Johnson County Hospital, Lexington Regional Health Center, Kearney Regional Medical Center, and Montgomery County Memorial Hospital.

Ashley Synowicki, DNP
Ashley started her health care career by attending Methodist College where she earned her Bachelors of Science in Nursing. While working at Methodist Hospital, she went back to school for her Doctorate of Nursing Practice (Family Nurse Practitioner) from Creighton University. She comes to us with years of advanced practice knowledge, expertise, and skills. Ashley is extremely versed in all thing's health and wellness. She prides herself on being incredibly organized, precise, and emerges herself in the latest research to help better her practice.
When not working incredibly hard, you can usually find Ashley on a walk with her husband and puppy, spending time with her family, or watching the Minnesota Vikings. Ashley enjoys family practice medicine because she believes that whole self-wellness is the key to living a long, happy, healthy life.
Christy Anne Ott, PA-C

Christy Ott is a board-certified Physician Assistant with over two decades of experience delivering patient-centered care across primary care, urgent care, and occupational health settings. She has formerly practiced with Nebraska Medicine Immediate Care Clinics and as a contracted provider with Signify Health, where she performed Medicare Wellness in-home evaluations.
Previously, Christy served for 13 years at Memorial Community Hospital & Health System in Blair, Nebraska, where she provided comprehensive care spanning family medicine, urgent care, and occupational health. She has also held long-term clinical roles at CHI Health in Missouri Valley, IA, and Southwest Family Physicians in Omaha, NE, consistently offering accessible, evidence-based care in high-volume environments.
Christy earned her Master of Physician Assistant Studies from the University of Nebraska Medical Center in 2000, following her Bachelor’s degree from the University of Nebraska at Kearney in 1998. Cindy Kathman, NP
Cindy completed her BSN in 2000 through Creighton University and her MSN through Clarkson College in 2017. She worked to formalize the palliative care program at Mary Lanning Health Care in Hastings. As the palliative care nurse practitioner, she saw patients at the hospital, nursing homes, ALFs, and in their homes within a 60-mile radius of Hastings. She has also been the in-house provider at Edgewood Assisted Living since 2020.
Cindy resides in Juniata, Nebraska, with her husband. They have three children and four grandchildren. In her spare time, she enjoys spending time with her grandkids and gardening. She commutes to Lincoln two days a week.
Deb Bussey, APRN
Deb Bussey is an Adult Gerontology Primary Care Nurse Practitioner, board certified by the ANCC. She began her nursing career over 33 years ago obtaining a Bachelor of Science in Nursing from UNMC. She has worked in a variety of healthcare settings including the past 10 years as a wound, ostomy and continence nurse. She strongly believes in lifelong learning and graduated in 2023 with a Master of Science Nurse Practitioner degree. She enjoys the challenges presented in working with the adult and geriatric population.
Deb resides in Lincoln with her husband and has 3 adult children living in the Lincoln and Omaha area. She enjoys spending time with family, outdoor activities, gardening, exercising, traveling, and caring for her dogs and cats.
Jenny L. Hackwell

Jenny is a part-time Wound Care Nurse Practitioner at Community Healthcare Partners, bringing with her a wealth of experience in wound care. She graduated from Purdue University in January 2021 with her APRN and has had a diverse career in nursing. From 2001 to 2021, Jenny worked as a Licensed and Registered Nurse in various settings, including hospital nursing, home health nursing, where she also served as the Director of Nursing, and as an Infusion Clinical Liaison. Since 2021, Jenny has been with Nebraska Medicine in Omaha, specializing as a Wound Care and Plastic Surgery Nurse Practitioner. She has a strong passion for patient-centered care and enjoys collaborating with healthcare professionals to deliver exceptional wound management and support throughout the surgical process.

Kara Walker

Kara Walker is a board-certified Family Nurse Practitioner with a diverse clinical background spanning urgent care, cardiology, and home-based health assessments. She currently practices at Heartland Urgent Care, where she delivers acute care services including patient evaluation and treatment, suturing, diagnostic interpretation (labs, radiographs, and testing), medication management, and STD counseling. She is proficient in Codonix EMR and excels in high-paced clinical environments.
Her previous roles include conducting in-home health assessments with Signify Health and serving as a cardiac Nurse Practitioner with Pioneer Heart Institute, where she managed hospital admissions, discharges, overnight calls, and inpatient rounding. During this time, she worked with both Epic and NextGen EMR systems.
Kara earned her Master of Science in Nursing (Family Nurse Practitioner) from the University of Nebraska Medical Center and her Bachelor of Science in Nursing from Kansas Wesleyan University. She is certified by the American Academy of Nurse Practitioners and holds active APRN and RN licenses in the state of Nebraska.

Lana Ekberg-Wood, FNP
Lana Wood is a native of Wakefield, Nebraska. After graduating from Wayne State College with a degree in Psychology, she attended UNMC and received her BSN going on to work inpatient Cardiology and in Home Health Care. In 2006, she enrolled at Clarkson College in Omaha where she earned her Master’s Degree in Nursing as a Family Nurse Practitioner. Lana has been a Nurse Practitioner in Omaha and Lincoln since 2009, specializing in Urgent Care, Home Health Care and Aesthetics. She is married and has one daughter.
Renee Schmidt, NP
Renee Schmidt is a dedicated Family Practice Nurse Practitioner who earned her Master’s degree in Family Nursing Practice from Clarkson College in Omaha in April 2019. With a diverse background, Renee has extensive experience in orthopedics, cardiology, geriatric and acute rehabilitation, long-term primary care, hospice, and women’s health. Prior to becoming a Nurse Practitioner, she worked as a scientist, bringing a unique perspective to her clinical practice.

Taylor Harding

Taylor Harding is an Adult Gerontology Primary Care Nurse Practitioner. She began her nursing career as a CNA/Med Aide while going through nursing school. She graduated from Nebraska Methodist College with her Bachelor of Science in Nursing. While working at Methodist Hospital, she went back to school for her Doctor of Nursing Practice. She enjoys taking care of the Older Adult Population and providing them with quality and effective care.

William Sumner, MSN, APRN, FNP-C
William is a board-certified Family Nurse Practitioner with over 13 years of combined clinical experience in emergency medicine, mental health, primary care, and military health systems. He holds a Master of Science in Nursing from Gonzaga University and is known for delivering high-quality, patient-centered care in both civilian and military settings.
William previously served as an Emergency Nurse Practitioner at Community Hospital in Fairfax, Missouri, managing a wide range of acute medical and trauma cases. He also has significant mental health experience, independently overseeing care for over 700 patients at Family Connections, Inc., through both in-person and telehealth services.
During his Navy service (2017–2021), William held key leadership roles, including Family Medicine Division Officer and Medical Emergency Manager at Naval Branch Health Clinic Everett, and Department Head of Deployment Health at Naval Hospital Oak Harbor. He led medical readiness initiatives for thousands of service members and played a vital role in launching the Navy’s MHS Genesis EHR system. He was honorably discharged at the rank of Lieutenant.
William holds certifications in ACLS, PALS, BLS, and ATLS, and is licensed as an APRN, RN, and CNP, with a DEA license and TS/SCI clearance.
