Our Facility-Based Program is specifically designed to provide Acute Rehabilitation and Long-Term Care facilities with expert patient oversight, rounding, and treatment options. Delivered through our team of Physicians, Nurse Practitioners (NPs) and Physician Assistants (PAs), our services are structured to function similarly to hospitalist programs, with a focus on the acute care management of rehabilitation patients. Our providers bring advanced skills in addressing the complexities of acute care while ensuring effective disease management and recovery. Additionally, our team specializes in primary and family care, offering Long-Term Skilled Nursing Facility residents the highest level of primary care and chronic care management. The foundation of our program includes comprehensive NP/PA services such as physical assessments, disease management, pharmacology, and patient education, ensuring robust delivery of primary care principles and optimizing both short- and long-term patient outcomes.

Physician Lead Team
CHP providers are supported and overseen by a dedicated group of experienced Hospitalist-based Physicians. These physicians play a critical role in guiding and supporting our providers' patient care delivery, offering expertise and direction on an individualized basis whenever necessary. This collaborative approach ensures that each patient receives tailored, high-quality care, grounded in the latest medical practices and standards.
Medical Director Options
CHP also offers the option to contract with individual facilities for Physician Medical Director Services, which complement our Provider Rounding Services. This integrated approach creates a comprehensive program that allows us to partner with facilities in managing key aspects of care, such as quality measures, patient outcomes, and star ratings.
